REED CITY — Blue Cross Blue Shield of Michigan has designated Spectrum Health Reed City Family Practice and Spectrum Health Evart Family Practice patient-centered medical homes. This means that Spectrum Health’s Family Practice sites are among a select group of primary care physicians in Michigan who are improving primary care in Michigan by adopting the PCMH model of care.

“The patient-centered medical home is health care centered on the patient,” said Family Practice medical director, Dr. Jonathan Dennis. “We take a holistic and comprehensive approach to patient care by helping patients meet their health goals and keeping track of all their specialist visits, test results and prescriptions.”

With the PCMH model, care teams are created that work with patients to improve health, keep them healthy and monitor their care on an ongoing basis.

Each family practice site coordinates your health care using registries to track conditions and ensure that patients are receiving the care they need. They are able to offer patients more access to the care team; coordinate complimentary care (such as nutrition counseling) and help patients better manage conditions like asthma and diabetes.

A recent analysis of Blue Cross claims data shows that PCMH designated doctors are succeeding in more effectively managing patients’ care to keep them healthy and prevent complications.
